Advances in Modern Dentistry: How Dental Care Has Changed Over the Last 20 Years

Dentistry has changed significantly over the last two decades. Treatments that once involved uncomfortable impressions, manually processed X-rays and lengthy planning can now often be completed using digital scans, three-dimensional imaging and computer-guided technology.

However, the transformation is not limited to equipment. Modern dentistry has also become more focused on patient comfort, communication, prevention and personalised treatment. Patients are now more involved in decisions about their oral health and have access to a wider range of restorative, cosmetic and implant treatments.

In a recent Abbey House Dental podcast conversation, Dr Amit and Dr Sandesh reflected on their careers and discussed some of the most important advances in modern dentistry over the past 20 years. Their discussion covered changes in NHS dentistry, digital technology, dental implants, cosmetic treatments, patient experience and the possible future of dental care.

Dentistry 20 Years Ago

Dental practices looked and operated very differently 20 years ago.

Appointments were often shorter and more clinically focused. A patient would enter the surgery, receive an examination, be told what treatment was necessary and leave shortly afterwards.

The main aim was usually to identify and treat disease. There was less discussion about improving the appearance of a smile, restoring long-term function or exploring different treatment options.

Many procedures also depended heavily on traditional materials and manual processes. Dentists commonly used physical impressions, film-based X-rays and conventional implant-planning methods.

Today, dentistry is generally more collaborative. Dentists still diagnose and treat oral disease, but they also spend more time understanding what the patient wants to achieve.

Changes in NHS Dentistry

One major change discussed in the podcast was the transformation of NHS dentistry following the introduction of a new contract system around 2006.

Before these changes, NHS dental care was more widely available through a system in which dentists were generally paid according to the individual treatments they provided.

The newer system introduced Units of Dental Activity, commonly known as UDAs. Dental practices were given activity targets and could face financial consequences if those targets were not completed.

Although the system was intended to encourage preventive care, many dentists believe it has remained heavily target-driven. It has also become more difficult for some patients to find an NHS dentist, resulting in increased demand and longer waiting lists in certain areas.

These pressures have contributed to the growth of private dentistry, where practices can offer longer appointments, additional treatment options and a more personalised patient experience.

The Move from Traditional Impressions to Digital Scanning

One of the most noticeable advances in modern dentistry is the replacement of traditional dental impressions with digital scanning.

Traditional impressions usually involve placing a tray filled with alginate or another impression material into the patient’s mouth. Although effective, many patients find the process uncomfortable, particularly if they have a sensitive gag reflex.

Digital scanners provide a more comfortable alternative.

A handheld scanner captures detailed images of the teeth and gums, creating a three-dimensional digital model of the mouth. The scan can then be sent electronically to a dental laboratory within minutes.

Digital scanning provides several advantages:

  • Greater comfort for the patient
  • Highly accurate dental models
  • Faster communication with laboratories
  • Reduced need for repeat impressions
  • Better-fitting crowns, bridges and aligners
  • Easier treatment planning and explanation

Because the digital model can be viewed immediately, the dentist can also identify missing areas and correct the scan while the patient is still in the surgery.

Faster and Clearer Digital X-Rays

Dental X-rays have also become much faster and easier to interpret.

In the past, X-ray films had to be manually processed using chemicals or specialised machines. Dentists sometimes had to wait 15 to 20 minutes before they could examine the result.

Today, digital X-rays can appear on a computer screen within seconds.

The images can be enlarged and adjusted for brightness or contrast, allowing both the dentist and the patient to see areas of concern more clearly.

Digital X-rays can help dentists identify problems such as:

  • Tooth decay
  • Bone loss
  • Infections
  • Impacted teeth
  • Problems beneath existing fillings or crowns

Displaying the image on a larger screen also improves patient communication. Rather than simply being told that treatment is necessary, patients can see the problem and understand the dentist’s recommendation.

Three-Dimensional Imaging and Implant Planning

Dental implant treatment has particularly benefited from advances in scanning and imaging technology.

Years ago, implant planning often depended mainly on standard two-dimensional X-rays. In more complicated cases, patients might need to visit a hospital or specialist imaging centre for a CT scan.

Modern dental practices may now have cone beam computed tomography scanners, commonly known as CBCT scanners, within the practice.

A CBCT scan creates a three-dimensional image of the patient’s teeth, jawbone and surrounding structures. It provides important information about:

  • The height and width of the available bone
  • The position of nerves
  • The location of the sinuses
  • The angle at which an implant should be placed
  • The condition of the surrounding teeth

This information helps the dentist plan implant treatment more accurately and safely.

In some cases, the CBCT image can be combined with a digital scan of the patient’s mouth. Special software can then be used to plan the implant position and create a surgical guide.

The guide helps the dentist place the implant according to the digital treatment plan.

More Advanced Full-Mouth Implant Treatment

Digital dentistry has also improved the planning and delivery of complex full-mouth implant treatments.

During the podcast, the dentists discussed a full-arch implant case that involved several different technologies.

Digital scans and CT images were combined to plan the treatment. A surgical guide and prosthetic stent were created digitally. After the implants were placed, photogrammetry was used to record their exact positions.

Photogrammetry uses specialised markers and scanning equipment to accurately identify the relationship between multiple implants.

The information can then be used to create a temporary or permanent bridge designed to fit precisely over the implants.

Three-dimensional printing can also be used to manufacture temporary restorations quickly, helping the patient leave with fixed teeth sooner than would have been possible using older methods.

These developments can make complex implant procedures more efficient, predictable and comfortable for patients.

Improvements in Dental Implant Materials

Implant technology has improved not only through digital planning but also through developments in materials and implant surfaces.

Modern dental implants are designed to integrate effectively with the jawbone. Changes in implant coatings and surface design may support faster and more predictable healing.

Digital impressions can also be used when creating implant crowns and bridges. This reduces the dependence on traditional impression materials and helps laboratories manufacture restorations that fit more accurately.

While successful implant treatment still depends on factors such as bone quality, oral hygiene, general health and professional planning, modern technology gives dental teams more information and control throughout the process.

The Growth of Invisalign and Cosmetic Dentistry

Cosmetic dentistry has become considerably more popular and accessible.

Clear aligner treatment, including Invisalign, was relatively uncommon when many experienced dentists began their careers. Today, clear aligners are offered by dental practices throughout the country.

They can be used to gradually move the teeth without relying on traditional fixed metal braces in suitable cases.

Another popular approach is the align, bleach and bond process:

  1. Align: Straighten the teeth using clear aligners.
  2. Bleach: Whiten the teeth.
  3. Bond: Use composite bonding to improve the shape or appearance of selected teeth.

This method can improve a smile while preserving more of the natural tooth structure than some traditional cosmetic procedures.

Patients are also more aware of treatments such as:

Social media, video calls and increased awareness of dental treatment have contributed to the growing interest in smile improvement.

However, cosmetic treatment should always begin with healthy teeth and gums. Appearance should not be improved at the expense of oral health or long-term function.

A Greater Focus on What the Patient Wants

The relationship between dentists and patients has changed.

Historically, dental care was often more dentist-led. The dentist identified the problem and decided which treatment was required.

Modern dentistry is generally more patient-centred.

The dentist explains the diagnosis, discusses the available options and helps the patient make an informed choice based on their needs, preferences and circumstances.

For example, a patient with a missing tooth may be able to consider:

Each option has its own advantages, limitations and costs. A patient-centred approach allows the individual to understand these differences before deciding how to proceed.

The Modern Dental Patient Experience

One of the biggest changes in private dentistry has been the increased focus on the overall patient experience.

Older dental practices were often designed mainly around clinical efficiency. Reception areas, lighting, decoration and comfort were considered less important than the treatment rooms themselves.

Modern practices may feel more like calm healthcare or wellness environments.

They may include:

  • Comfortable reception areas
  • Softer lighting
  • Modern furniture and interior design
  • Calming scents or music
  • Patient treatment coordinators
  • Digital consultation screens
  • Clearer communication before and after treatment

These details are not intended to replace high-quality clinical care. Their purpose is to help patients feel more relaxed and supported.

Dental anxiety is common, and the environment can strongly affect how a person feels about attending an appointment. A welcoming practice, helpful staff and clear explanations may make it easier for nervous patients to receive the care they need.

The Role of Treatment Coordinators

Patient treatment coordinators have also become a more common part of the dental journey.

A treatment coordinator can speak with the patient, explain how the treatment process works and help organise appointments.

Depending on the practice, they may also:

  • Discuss the stages of treatment
  • Arrange digital scans
  • Explain payment or finance options
  • Answer non-clinical questions
  • Follow up after consultations
  • Help patients understand what happens next

Clinical decisions must still be made by appropriately qualified dental professionals. However, treatment coordinators can give patients more time and support than may be available during a clinical appointment alone.

The Expanding Role of the Dental Team

Modern dentistry is no longer delivered only by dentists and dental nurses performing traditional roles.

Many dental professionals now receive additional training and take on greater responsibilities.

The wider dental team may include:

  • Dental hygienists
  • Dental therapists
  • Orthodontic therapists
  • Clinical dental technicians
  • Dental nurses with additional qualifications
  • Treatment coordinators
  • Implant coordinators

Depending on their qualifications and professional scope, members of the dental team may be able to take X-rays, perform digital scans, provide fluoride treatments, carry out hygiene care, complete certain fillings or support orthodontic treatment.

This team-based approach allows dentists to concentrate on diagnosis, complex restorative treatment, surgery and other advanced procedures.

It may also improve access to preventive care and make the patient’s treatment journey more efficient.

Prevention and Patient Education

Although new technology often receives the most attention, prevention remains one of the most important areas of modern dentistry.

Digital scans can help patients see areas where their oral hygiene may need improvement. Dentists and hygienists can enlarge images, compare scans over time and explain changes in the teeth and gums.

Some systems can also create visual reports or three-dimensional models that show:

  • Gum recession
  • Tooth wear
  • Changes in tooth position
  • Areas where plaque may accumulate
  • Possible changes in the bite

Showing the patient what is happening can be more effective than giving instructions alone.

When patients understand the condition of their mouth, they may feel more motivated to brush effectively, clean between their teeth and attend regular examinations.

How Modern Dentistry Can Reduce Dental Anxiety

Many of the advances discussed throughout the podcast can help make dental treatment less stressful.

Digital impressions avoid the discomfort associated with traditional impression trays. Faster X-rays reduce waiting time. Three-dimensional implant planning provides clearer information before treatment begins.

Modern practices also place greater importance on communication and comfort.

A patient who knows what will happen, understands why treatment is recommended and feels able to ask questions is likely to feel more in control.

Technology cannot completely remove dental anxiety, but it can support a more predictable, transparent and reassuring experience.

The Future of Dentistry

Dentistry will continue to develop.

Artificial intelligence is already influencing areas such as image analysis, digital treatment planning and administrative processes. It may eventually help dentists identify patterns in X-rays or scans and support earlier diagnosis.

Robotics may also play a role in future dental procedures, although widespread use in everyday practice is likely to take time.

Other possible developments include:

  • More advanced 3D printing
  • Faster production of crowns and bridges
  • Improved digital implant planning
  • More accurate artificial intelligence-assisted diagnosis
  • Regenerative treatments
  • Biological tooth replacement
  • Increasingly personalised dental care

Researchers are exploring ways to repair or regenerate damaged tissues. Although growing replacement teeth or using robots for routine dentistry may sound futuristic, continuing developments in biology and technology could eventually change what is possible.

The dentist’s role is unlikely to disappear. Instead, technology will probably provide dental professionals with better tools to diagnose conditions, plan treatment and care for patients.

Are We in a Better Place Than 20 Years Ago?

Dentistry still faces significant challenges, particularly around access to NHS treatment, affordability and workforce pressures.

However, from a clinical and technological perspective, dental care has advanced considerably.

Digital scanning has made impressions more comfortable. Digital X-rays provide faster and clearer images. CBCT scanning has improved implant planning. Clear aligners and modern cosmetic treatments have become more widely available.

At the same time, dentistry has become more patient-centred. Practices now place greater importance on communication, comfort, informed consent and the overall treatment experience.

The most meaningful advances in modern dentistry are therefore not simply new machines or techniques. They are improvements that help dental professionals provide safer, more accurate and more personalised care.

The technology may continue to change, but the central purpose of dentistry will remain the same: protecting oral health, restoring confidence and giving every patient a reason to smile.
